The impact of information and communication technology (ICT) is indirect and depends on redesign of practices and structures also outside health care. Improvements will only be realized if all parties involved can coordinate their efforts to take advantage of new technology. A 'package' of changed work practices and structures extending across organizational boundaries needs to be designed and implemented. This is very different from the common conception of introducing new ICT tools. Calls for 'evaluation of benefits' before new ICT systems are introduced need to recognize this complexity. This article investigates how analysis and economic evaluations can be used to improve decision-making when new applications are proposed. This is done by drawing parallels with experiences from other industries. We conclude that the entire 'change package' should be analysed for its consequences on the well-being of care recipients, and the requirements it presents for capital investments and changed labour inputs,in particular changed competence needs. Some concepts and structures are suggested for such evaluations.

This research aims to study the relationship between components of enterprise architecture management (EAM) and its impact towards IT benefits and business benefits. This research was conducted using quantitative approach involving 92 samples of state-owned enterprises and private enterprises. The data were processed using partial least square (PLS). The results showed that the EAM was proven to have positive correlations on the IT benefits especially towards IT efficiency and IT-business strategic alignment. Subsequently, IT benefits were proven to have positive correlations towards business benefits especially towards customer closeness and product leadership. It was also found that EAM has positive correlations on business benefits through IT benefits, especially towards operational excellence and strategic intelligence. Thus, it can be concluded that EAM can improve business performance through improved IT performance.

Measuring the lifetime costs and benefits of medical technologies is essential in evaluating technological change and determining the productivity of medical care. Using data on Medicare beneficiaries with a heart attack in the late 1980s and 17 years of follow up data, I evaluate the long-term costs and benefits of revascularization after a heart attack. I account for non-random selection into treatment with instrumental variables; following McClellan, McNeil, and Newhouse, the instrument is the differential distance to a hospital capable of providing revascularization. The results show that revascularization is associated with over 1 year of additional life expectancy, at a cost of about $40,000. Revascularization, or other treatments correlated with it, appears to be highly cost-effective.

The year is 2006, and there are just a few hundred medical transcriptionists (MTs) still transcribing reports, serving only those older physicians who haven't changed with the times--and the times have definitely changed. Physicians have finally recognized the power of electronic health records (EHRs) as well as the fact that this power is realized only if they input clinical data directly into the EHR. The vast majority of physicians are using empirically refined templates, pick lists, and other methods of structured, codified input through the evolved progeny of today's Palm PCs, Pocket PCs, and Tablet PCs. Input methods include touch-screen, speech recognition, handwriting recognition, and perhaps other technology not yet invented. There are no longer any delays or expenses resulting from transcription. Plus healthcare organizations enjoy numerous benefits derived from analyzing codified clinical data. But this is only one vision. Another vision of 2006 incorporates an unavoidable reality: manyphysicians strongly resist directly inputting clinical data. They believe it slows them down, which outweighs the potentials overall healthcare benefits. Additionally, these physicians believe that structured input of patient information limits the freedom of expression afforded by free text. And frankly, these physicians don't put much stock in the value of clinical practive analysis. So transcription continues. In fact, it expands dramatically. Due to regulatory controls and other pressures, more providers dictate more clinical notes than ever. The need for MTs explodes. In 2006, there are half a million MTs required to convert voice dictations into text, more than double today's number.

The use of information technology (IT) in the built environment has numerous benefits, including improving performance, sustainable practices and efficiency on projects. Studies have investigated the measurements of various IT benefits, but a comprehensive review of the benefit analysis methods used to produce quantifiable measurements of IT benefits in the built environment to provide evidence is still lacking. The study's aim is to systematically review the benefits analysis methods used to produce quantifiable evidence of IT benefits in the built environment. Through a three-stage review process, relevant papers were selected and examined based on the benefits analysis methods and the results produced. The findings show that nine benefit analysis methods are used to measure the benefits of IT which could be tangible benefits or intangible benefits. The results indicate that the benchmarking and weighted scoring models are the most popular analytical method used to produce quantifiable results of benefits measured. The shortfalls of the benefit analysis methods are discussed, and future research agenda is recommended to ensure comprehensive results from benefit measurements, Further, a conceptual integration model is developed to guide future research when addressing the shortfalls of the respective benefit analysis methods through the stages of implementation of IT.

Purpose - Financial measures, available aplenty, of returns on investments in information technology (IT) are deemed insufficient in terms of inclusion of intangible benefits. The purpose of this paper is to re-conceptualize the IT business value (ITBV) benefits by Gregor et al (2006) after a contemporary evaluation. Design/methodology/approach - IT experts have been administered the instrument to provide insight into the contemporary cogitation of ITBV benefits and factor analyses are employed to re-conceptualize the ITBV benefits. Findings - Of the four ITBV benefits, the statistical analysis suggests that Informational and Transactional benefits need to be re-conceptualized. It is affected through a systematic linking of parameters - business strategy, objectives, information systems (IS) strategy, IS role and IS-business alignment profiles in that order. Research limitations/implications - Operational definitions of re-conceptualized ITBV benefits have been presented which may be operationalized into components. Practical implications - The re-conceptualization provides managers with an enhanced understanding of ITBV in line with the alignment of IS with business objectives and strategy and guides managers to develop customized ITBV measures. Originality/value - The study employs factor analyses to analyse the ITBV benefits. It proves that the changing face of IT, wherein it is looked at as a value creator, needs an all-encompassing and contemporary envisioning of ITBV benefits. It also suggests an advanced typology based on further insightful IS literature.

Healthcare spending will exceed Dollars 4 trillion by 2017, a trend that is leading executives to implement information technology (IT) systems to contain these rising costs. Studies show that numerous factors determine the outcome and net benefits of IT in healthcare. However, what happens when a newly implemented IT system results in negative outcomes? We explore this question by examining a newly implemented IT system in a large hospital that was yielding none of the benefits for which its designers had hoped. Using an expanded set of analytic lenses, our in-depth study found that political issues were a major stumbling block to the implementation of this IT system, as the interests of IT managers were different from those of the system's users. In addition, cultural values among these stakeholders were not aligned. The new IT system carried very different meanings for these two key groups. These political and cultural issues, which reflect a broader set of factors than is commonly applied in IT or in management, led to specific recommendations designed to improve the system's viability and benefits. In a follow-up analysis we found that these alternative lenses helped increase the intended usage of the IT system by 16 percent in the first year, yielding a 20 percent improvement in performance. By better understanding the cultural and political significance of IT implementation, managers may thus improve the effectiveness of new information technologies for containing costs in hospitals.

The Voice Assistant (VA) market is emerging rapidly. Considering the unique features of voice technology, primarily with respect to its ability to provide a total hands-free way of communication, existing technology acceptance models might not be comprehensive enough to explain the users' attitudes towards using this technology. Moreover, extant research on VAs are fragmented, with two separate directions. The first one takes a technology acceptance based approach, whereas the second one takes a user satisfaction based approach for explaining the usage of the VAs. In this work a comprehensive model is proposed by integrating the two separate research directions together incorporating the concepts from Technology Acceptance Model (TAM) and Wixom & Todd Information System Success Model (W&T ISS). Data is gathered using an online survey from 419 people, and the results are analyzed using the Partial Least Squares Structural Equation Modelling. Results show a statistically significant positive association between the object-based beliefs and attitudes (Information Quality, System Quality, Service Quality) with the behavioral-based beliefs and attitudes (Perceived Usefulness, Perceived Ease of Use, Perceived Enjoyment). Among the two other contextual factors the effect of privacy risk is found to be non-significant, whereas that of perceived compatibility is significant. Overall, the proposed research model has a good fit (R-2=64.6%). Based upon the results, appropriate theoretical and practical implications are discussed.
